Office Inspection :
Prior to obtaining an unrestricted permit , an applicant ’ s office must pass an inspection conducted by the Board through its authorized agents in accordance with the AAOMS Manual ( for OMS ), ADA Guidelines ( for non-OMS ) and AAPD Guidelines for pediatric patients . Re-inspection will occur every six ( 6 ) years .
Clinical Evaluation :
Prior to obtaining an unrestricted permit , an applicant must pass a clinical evaluation conducted by the Board through its authorized agents in accordance with the AAOMS Manual ( for OMS ), ADA Guidelines ( for non-OMS ) and AAPD Guidelines for pediatric patients .
Duties :
• Must ensure that a patient medical history is taken or updated
• Must give patients a physical evaluation
• Must have proper equipment
• Must have signed patient consent
• Pediatric patients administered anesthesia or deep sedation by or under non-OMSs must be done by a person solely responsible for monitoring , while another dental licensee not responsible for monitoring the patient will perform the dental work
• Must obtain 15 hours of continuing education credits in courses related to general anesthesia or deep sedation every licensure cycle . This counts toward the overall 30-hour CE requirement .
Auxiliary Personnel :
• Must be properly trained for delegated duties
• Must perform duties under direct onpremise supervision of the permit holder
• Must be certified in BLS
• Nurse anesthetists must perform their duties under direct on-premise supervision , perform duties that are under scope of practice and maintain ACLS certification .
Restricted Permit I Holders
( conscious sedation , nitrous oxide / oxygen analgesia )

Requirements :
A permit holder must do one ( 1 ) of the following :
1 . Successfully complete a course on conscious sedation comprising of at least sixty ( 60 ) hours of undergraduate or postgraduate didactic instruction and clinical experience that conforms to Part I or Part III of the ADA Guidelines .
Certification :
OMS and Non-OMS 1 . Must have certification in ACLS to administer to adults .
2 . Must have certification in PALS to administer to children ( under 18 ).
Standards :
OMS
1 . Must attest that the administration of anesthesia / sedation conform with AAOMS Guidelines and Manual .*
Non-OMS
1 . Must attest that the administration of anesthesia / sedation to adults conform with ADA Guidelines . 2 . Must attest that the administration of anesthesia / sedation to children ( under 18 ) conform with AAPD Guidelines .
* Conflicts between the AAOMS Guidelines , the ADA Guidelines or the AAPD Guidelines , and the Board ’ s regulations , will be resolved in favor of the applicable Guidelines .
