Certain focus areas by the DMR Inspectorate were identified during this year and will definitely continue throughout next year; the identification and mitigation of all possible risks with regards to TMM-to- TMM and TMM-to-pedestrian accidents and incidents and the compliance to the applicable regulations. This will require the investigation of possible control measures, that is Proximity Detection Systems (PDS), and the continuous updating of traffic management plans. A further point of focus was following up on employees’ understanding of their legal responsibilities as stipulated in their legal appointments, COPs and operating procedures. This is definitely challenging for certain operations where the literacy levels of employees are very low. The main focus of the ISHE Audit is the evaluation of the implementation of the documented systems, the condition of the facilities and installations at the mine. This industry is not contributing to the list of fatal accidents in the mining industry, and that in itself is a major achievement towards Zero Harm. Some two decades ago somebody came up with the initiative of behaviour-based safety and this still makes complete sense with regard to achieving the objectives of the ISHE Audit. We would rather focus on solving potential safety risks, than to focus on the problem – or the person. History has shown us that people got fired if they were involved in an incident or accident. Focus should rather be to learn from their mistakes and put actions in place to prevent such accidents or incidents taking place again. This is the only solution to get to Zero Harm and to empower employees to take responsibility for their own tasks. The ISHE Audit is more than just an audit, but an activity where members are educated in what is required to be legally compliant and how to achieve this.
