• First-time grant recipients

• Past monitoring findings and concerns , if unresolved

• Lack of accuracy and timeliness of monthly reports

• Expenditure / performance rates ( if not within 10 % of target )

• Staff turn-over ( key positions )

• Multiple grants ( CDBG , ESG , HOME )

• No on-site monitoring visit for > 2 years
