SELF CERTIFICATION FORM ( CDBG )

• ALL parts of the form must be completed

• Hispanic is classified by HUD as an Ethnicity NOT a race

• Mark Hispanic for Ethnicity

• Both the Client and an Organization Representative MUST sign and date

• New form must be completed by clients at the beginning of each new program year for each grant
