too. Or indeed at any time we attempt to freeze data, the shape of the service and
related issues in a fixed point in time.
So, although we endeavour to keep the detail and data as fresh as we can, this book is
intended to be much more than a factual document. It is intended to encourage you to
think and equip you to participate in the system in whatever way you see fit.
We will consider the key drivers of organisational change, the conflicting challenges
facing the NHS and the different approaches being taken by each of the four home
nations of the UK. We will continually relate the ‘big’ topics to everyday leadership for
doctors and clinicians of all disciplines. If you believe in the moral and professional
responsibility to understand the system then this is an ideal place to start or to refresh
your thinking. The next challenge is to apply your knowledge and keep it up-to-date.

1.02 Your current perceptions
As you work through this book you will be encouraged to clarify and make your personal
thoughts regarding the NHS and your current circumstances explicit. This should place
you in good stead for being able to discuss topical issues with others as and when the
need arises. So, let’s start by thinking of a few words which describe your thoughts and
feelings about the NHS.
