OCF 18 Treatment Plan
The OCF 18 Treatment Plan is completed by health care providers to provide a guideline to insurers
regarding:
cause and nature of injuries resulting from a motor vehicle accident
identify limitations
identify treatment plan and goals
prior and concurrent conditions
proposed treatment and estimated costs
increase accountability of all parties involved.
Click onto the New Accident button. Click Yes, then OK.
Click the New Treatment Plan (OCF18) button. The form will open with the all the parts in tabs across
the top. Click on any tab to go to that specific tab.
