Step 2:  MTSS Team 

  • During MTSS Team meeting, review data of scholars who were identified by teachers as needing additional support to determine if action plan is needed 

  • Brainstorm scholar-specific interventions and create an Action Plan

  • Focus on interests and motivators for scholars while creating plan

  •  Step 3:  Tracking & Cultivating Positive Relationships

  • Family contact upon creation of the Action Plan  (there should be a positive contact before this)

  • Share with family the additional support being provided

  • Link the Action Plan to the School MTSS Tracking Sheet

  • Step 4:  Implement Interventions with Fidelity 

  • Implement documented interventions for 8-10 weeks

  • Monitor growth and document progress monitoring data on the Action Plan

  • Step 5: Review scholar growth after 8-10 weeks.

  • If adequate growth is made: Continue to implement interventions with fidelity and monitor growth

  • If scholar is not showing adequate growth after 8-10 weeks of interventions being implemented with fidelity: Scholar growth data and action plan reviewed by MTSS Team and teacher to determine new interventions, next steps, or potential referral for testing

  • Step 1:  Universal Screening and Data Collection

  • All scholars are screened with an academic universal screener (Dibels, NWEA, iReady, etc.) to identify scholars who are in need of academic support

  • Behavioral data collected for all scholars to determine scholars in need of behavioral support

  • Based on cut-off scores from the universal screener, classroom data, and behavioral data, teachers create a list of scholars who may need individualized support/action plans and share this list with MTSS Team
