Many British Columbian residents are now aware of the simplicity of a name change . Everyone who wishes to change their name from what is originally in their birth certificate must apply to change their name . This requirement involves the completion of a document called Application for Change of Name and it can be obtained from the Ministry of Health and Ministry Responsible for Seniors ( Vital Statistics Agency ). To make an application to change your name or to change your child ’ s name , the applicant must be at least 19 years of age . If you are under 19 years of age , then you must have your parent make the application on your behalf .
Many of my clients are not aware of the following :
1 . You do not need a lawyer to change your name . You can do it yourself .
2 . If you are

Sa Taong 2017 ay magbibigay ng bagong hamon at bagong pag-asa para sa One Filipino Cooperativre of BC para maabot ang layuning makapagbigay ng serbisyo sa mga kasapi pati na sa mga kababayang Pinoy na matugunan ang mga pangangailanan hindi lamang pang-pinansiyal o pang sosyal kundi para sa kabuuang pagkatao .
Sa taon ding ito makakapagbigay ng malaking pagtuon ang isa sa pangarap na programa hindi lamang ng mga FilCo-op Members kundi pati na ng nakakaraming kababayan na makapagtayo ng isang Housing Cooperative for Filipinos . Ang pangarap na proyektong ito ay nakasama sa Medium Term Plan ng FilCo-op at sinimulan seryosohing tutukan noong nakaraang Oktubre 2016 . Naformalize ito sa pag-apruba
getting married , you can choose to continue using your own surname or you can start using your spouse ’ s surname . This does not require an application or a legal change of name under the Name Act .
3 . A spouse by marriage may use the surname he or she had immediately before the marriage .
4 . A spouse by marriage may use the surname he or she had at birth or by adoption .
5 . A spouse by marriage may use the surname of his or her spouse by marriage .
BUT if you wish to use a hyphenate surname – you HAVE to apply for a legal change of name . In addition :
6 . I n d i v i d u a l s who decide to return to their original surnames at a later date do not need to apply for a change of name .
7 . In the case

Sa kasalukuyan ay patuloy ang mga Programa ng FilCo-op para makatulong pa sa ating mga kababayan . Pangunahin dito ay ang PAHIRAMAN NG BAYAN na tumutugon sa pangangailangang pinansiyal at tulong sa pagpupursigeng makapag-ipon para may magamit sa panahon ng pangangailanan . Kabilang sa mga serbisyo ay ang regular loan , emergency loan , car loan , bayad sa credit card loan at membership share
Name Changes Keeping Your Identity or Taking Your Spouse ’ s Name
of a divorce or a person just wanting to revert to a previous name , they can choose the following : their married surname , their previous married surname or their name at birth .
If you do not fall in any of the above categories and you merely want to change your name , then you must apply to have your name changed legally . This requires a step by step process and requires a number of documents .
( a ) a published Notice of Intention to change your name in your local newspaper . The original published notices with name of publication and dates published ;
( b ) original birth certificate if born in Canada ;
( c ) if born outside of Canada , a photocopy of immigration documentation ;
( d ) a p p l i c a n t ’ s original certificate of marriage , if married in Canada ;
( e ) if married

( f ) if you are changing your child ’ s name , proof of custody AND consent of the other natural parent
( g ) the required fee .
You are free to choose any new name you wish but the Director of Vital Statistics has the right to refuse any application if the new name will cause confusion or embarrassment . By law , you must have a first and last name otherwise your application will be refused .
Once your application is approved , the Division of Vital Statistics will automatically change the name on your birth record ( if born in B . C .) and marriage certificate ( if born in B . C .).
