Pre-Admission Testing
� Your surgeon ’ s office will let you know if you need testing before the day of surgery .
� If your surgeon does your history and physical , please have their office fax the results to 614-234-8231 . If you have questions , please call the Mount Carmel Pre-Admission Testing office at 614-663-2560 during business hours .
� If you are scheduled with our Pre-Admission Testing ( PAT ) department , you will get a separate appointment time from your surgery .
� Appointments take about 1 to 1.5 hours to complete .
� Pre-Admission testing is on the Mount Carmel Grove City campus at :
5300 North Meadows Drive , Building 2 , Suite 1800 Grove City , Ohio 43123
� Free parking is available in the garage and parking lot across from the Building 2 entrance .
