Please let us know if you have ideas or suggestions to improve our programming.” A message from Jodii in Social Services… On an annual basis, we provide residents with an updated copy of resident rights and rules and regulations for the facility. If you would like a copy, please notify Social Services and one can be mailed or e-mailed to you. You may also pick up a copy from the information center in our front lobby. According to our federal regulations, all residents have the right to formulate an Advanced Health Care Directive (AHCD). An AHCD allows individuals to make their health care wishes known (such as which type of medical treatment they would refuse or accept). This directive requires notarization, but does not need to be done by an attorney. Anyone completing an AHCD must be able to understand the document’s purpose. This document is completed in advance of any need, so loved ones and physicians know your wishes in the event you are not able to make your wishes known. The forms are available at the facility and can be completed, notarized and extra copies provided at no cost.
