School-Based Concussion Case Manager
In the Junior and Senior School , one of the Learning Resource teachers will act as a case manager . Crofton House School has prepared an example of a case management plan . It is the responsibility of the school-based concussion case manager to :
• Be the link between home and school
• Track the recovery of the student
• Guide teachers , coaches and co-curricular supervisors in supporting the student through recovery
• Adjust expectations related to learning , homework , assessment and / or participation
Administrative Assistants
An administration assistant is often the first to receive the information regarding a concussion . It is the responsibility of the school administrative assistants to :
• Notify the school-based concussion case manager
• Forward to the school-based concussion case manager a copy of any attendance or medical documents
• Maintain documentation on file for attendance audit purposes and forward a copy of any attendance or medical documents to the school-based concussion case manager
Homeroom Teacher ( Junior School )
Teachers are part of the planning team . It is the responsibility of the homeroom teacher :
• Monitor a student ’ s recovery
• Adjust work / assessment expectations
• Implement a variety of learning strategies to help meet the concussed student ’ s learning needs as required
• Support the student as she “ returns to learn ” and , where appropriate , “ returns to play ”.
• Help identify changes in a student and notify the team of any concerns / news of progress that they see
Head of Grade and Student ’ s Advisor ( Senior School )
The advisor knows the student and her parents / guardians . As such , the advisor will play a key role in the student ’ s recovery and “ return-tolearn ” plan in collaboration with the concussion case manager . The advisor will be supported in this role by the Head of Grade . It is the responsibility of the Head of Grade and the student ’ s Advisor to :
• Monitor a student ’ s recovery
• Communicate with the student ’ s teachers , as required
• Support the student as she “ returns to learn ” and , where appropriate , “ returns to play ”

Administration of Student Medication

The School has developed a protocol for administering prescription or non-prescription medication . Depending on the student ’ s age and circumstances , it may be prudent for the School to manage the administering of prescription or non-prescription medication . On an overnight excursion , parents / guardians must inform the excursion coordinator of medication prescribed to a student . The excursion coordinator will assume or assign to another excursion supervisor responsibility for the administration of mediation , according to parent / guardian instruction .
