Reporting Concerns of Abuse and / or Policy Violations

The U . S . All Star Federation is committed to supporting a safe and positive environment for participants ’ physical , emotional and social development and promoting an environment free from abuse and misconduct . Through increased awareness , training and educational opportunities , we strive to remove barriers to reporting .
FOR ISSUES CONCERNING
• Sexual abuse
• Physical abuse
• Neglect
HOW TO REPORT : STEP 1 Contact law enforcement or local child welfare agencies designated to receive and investigate reports of suspected child abuse and neglect : STATE CHILD ABUSE AND NEGLECT REPORTING NUMBERS .
STEP 2 Complete the ONLINE REPORTING FORM . After contacting law enforcement , please have the following information available for your report to USASF : 1 . the entity to which you reported ; 2 . the name , phone number and / or email address of the person you spoke with or to whom the case has been assigned ; 3 . any case number or reference number you were given ; 4 . the date you made the report ; and 5 . If you were provided any documentation from the report , please have that available to upload .

• Sexual misconduct
• Physical misconduct
• Emotional misconduct
• Bullying , harassment or hazing
• Stalking
• Misconduct related to reporting
• Misconduct related to USASF ' s process
• Criminal charges or dispositions involving child abuse or sexual misconduct
• Violation of Abuse Prevention Policies ( APP )
• Other inappropriate conduct as defined in THE CODE OF CONDUCT & COMPLIANCE

FOR ISSUES CONCERNING :
• USASF age grid or roster violations
• Sanctioned event requirement violations by clubs , owners , coaches , athletes or personnel
• Sanctioning standard violations for events hosted by Event Producer members
• Other compliance policy issues outlined in THE CODE OF CONDUCT & COMPLIANCE ( pages 56-59 ).
• NOTE : You must be a current member of the USASF to file reports of noncompliance .
