Reporting Concerns of Abuse and / or Policy Violations
The U . S . All Star Federation ( USASF ) is committed to supporting a safe and positive environment for participants ’ physical , emotional and social development in an environment free from abuse and misconduct . Through increased awareness , training and educational opportunities , we strive to remove barriers to reporting .
HOW TO REPORT issues concerning :
• Sexual Abuse
• Physical Abuse
• Neglect
REPORT : STEP 1 Contact law enforcement or local child welfare agencies designated to receive and investigate reports of suspected child abuse and neglect : State Child Abuse and Neglect Reporting Numbers if you need assistance making a report to local authorities , our partners at Safe @ AllStar can assist . Call the Helpline at 720.282.1744 ( 8am-8pm EDT ).
REPORT : STEP 2 Complete the USASF / USA Cheer ONLINE REPORTING FORM . After contacting law enforcement , please have the following information available for your report to USASF :
• The entity to which you reported
• The name , telephone number and / or email address of the person you spoke with or to whom the case has been assigned
• Any case number or reference number you were given
• The date you made the report
• And , if you were provided any documentation from the report , please have that available to upload
HOW TO REPORT issues concerning :
• Sexual misconduct
• Physical misconduct
• Emotional misconduct
• Bullying , harassment , or hazing
• Stalking
• Misconduct related to reporting
• Misconduct related to USASF ' s process
• Criminal charges or dispositions involving child abuse or sexual misconduct
• Violation of Abuse Prevention Policies ( APP )
• Other inappropriate conduct as defined in the Code of Conduct & Compliance

HOW TO REPORT issues concerning :
• USASF age grid or roster violations
• Sanctioned event requirement violations by clubs , owners , coaches , athletes or personnel
• Sanctioning standard violations for events hosted by Event Producer members
• Other compliance policy issues outlined in the CODE OF CONDUCT & COMPLIANCE ( p 36-39 )

* You must be a current member of the USASF to file reports of non-compliance .

• Financial disputes
• Employment concerns
• Facility cleanliness
• Equipment issues
• Scores
• Competitive progression concerns / team placement
• Business practices between the consumer and provider
