CARDIAC REHABILITATION
HIGH-INTENSITY , SPORT-SPECIFIC CARDIAC REHAB TRAINING OF A COMPETITIVE CYCLIST AFTER SCAD
Spontaneous coronary artery dissection ( SCAD ) is an increasingly recognized cause of acute MI that tends to affect young , presumably healthy individuals . A 22-year-old man with SCAD wanted to assess the feasibility of returning to competitive cycling . He was referred to the cardiac rehab ( CR ) program at Baylor Scott & White Heart and Vascular Hospital – Dallas where the staff designed a high-intensity , sport-specific training program that simulated the movements and forces associated with his goal activity .
The program was symptom limited and enabled the patient to train earlier and at a higher intensity than is typically allowed in conventional CR programs . Daily exercise training was customized to match the physical demands of competitive cycling by using a road bike , an indoor bike power trainer and an interactive indoor-cycling software program . He completed the high-intensity exercise-training program without adverse signs or symptoms . The patient reported feeling more confident about resuming competitive cycling in an outdoor setting .

“ Leap ” stands for Lifestyle Education Awareness Program . For the past eleven years , this successful program has focused on the needs of cardiovascular patients and their family members . Leap for Life sessions in Dallas and in Fort Worth taught what steps can be taken to manage cardiovascular disease and to improve health and well-being . This wellness and disease prevention program was available to all heart patients and their family members . The class was also open to anyone from the community . Educators with special expertise in cardiovascular patient care helped empower individuals with physical , dietary and stress management education and information .

At four years post-SCAD , the former patient is symptom-free and is regularly cycling 200 to 250 miles per week .
