GETTING TO ZERO READMISSIONS
How a focused multidisciplinary team can create change
Completing approximately 200 First Rib Resections per year, Baylor
Hamilton Heart and Vascular Hospital volumes are among the highest
in the country.
When the Baylor Hamilton Heart and Vascular nursing and quality
department identified a trend of readmissions in the First Rib
population, largely due to pain and pleural effusions, the team was
able to significantly improve the quality of care for these patients by
creating a care-path that increased comfort and decreased pulmonary
complications post-operatively. Reducing complications improved the
patients’ acute stay and also enhanced patients’ quality of life long after
discharge. Furthermore, this initiative increased patient satisfaction with
pain management.

The baseline First Rib readmission rate for Quarter 3 of 2013 was
15.7%. Interventions took place from July 2013 through June 2015.
Readmissions for the First Rib patient population were decreased to zero
by July 2015 and have been sustained at zero since then.
