Through our Transformative Care Continuum , you ’ ll partner with communities to provide family medicine where it ’ s needed most , beyond the exam room . You ’ ll be rooted in your clinical teams – Cleveland Clinic Lakewood Family Health Clinic or Cleveland Clinic Akron General Center for Family Medicine – within your first days of medical school , staying in one clinical team through Heritage College graduation , family medicine residency , and board certification .
