Providing assessment and management of service users who present with physical , learning or mental health needs that impact on their ability to manage within their home environment . You will identify any potential for improvement via a collaborative approach across health and social care , reablement programmes , equipment , adaptations and Assistive Technology . You will support the longterm needs of service users who require major adaptations to their home environment , using your knowledge of function and the impact of the built environment to develop solutions to meet their needs and maintain independence .
