A new approach to personalised care in Sheffield
A newly-published case study looks at the work of the Personalised Care Team in a neighbourhood in the North-East of Sheffield , comprising seven GP surgeries . It explores the neighbourhood ’ s approach to developing a personalised healthcare system that brings together the combined experience of the health and voluntary sector and looks at what working in this way has achieved , some of the challenges encountered along the way , and what is required to sustain and build the work of the team . This case study forms a part of wider work being carried out across Sheffield with the Connecting Health Communities programme . It aims to demonstrate what can be achieved when strong relationships are formed in local partnerships between health and voluntary sector workers to address local needs . Social Prescribing Day 2023
Social Prescribing Day , taking place this year on 9 March , is an annual celebration of social prescribing , recognising link workers , local community groups and regional and national organisations that support people ’ s health and wellbeing . Resources for primary care teams
NHS England has launched a series of resources for primary care teams , including occupational therapy . It includes an A5 leaflet and A4 poster , social media and digital screens . Scottish Government and COSLA set out joint commitment to improve social care system
The Scottish Government and COSLA have set out their joint commitments to bring about improvements to the social care system over the next 12 to 18 months .
Adult social care 2022 to 2023 : joint statement of intent and next steps , builds on their original joint commitments , published in March 2021 , and places a fresh focus on addressing the twin impacts of the pandemic and the cost crisis .
The new statement sets out shared areas of focus across key parts of the social care sector for the Scottish Government and local government in the coming year . The joint aim continues to be to improve the experience and outcomes of the people who access social care support and those who work in the sector .
In the published statement , the Scottish Government and COSLA said : ‘ We have made solid progress towards delivering the commitments in the Statement of Intent , but we cannot stop there . The ongoing effects of the pandemic continue to place increased demands on the social care sector .
It is vital that we continue to strengthen and improve the delivery of social care support on an ongoing basis , and we must work together to develop and deliver the changes that are needed .”
‘ Since the publication of the first joint Statement of Intent , the cost crisis has become increasingly widespread , and it has made the need for sustainable improvements to the social care sector even more pressing .
‘ These commitments can be delivered now within the strong partnerships that exist locally and nationally . It is vital that we continue to strengthen and improve the delivery of social care support on an ongoing basis , and we must work together to develop and deliver the changes that are needed .’
One of the pledges for the year ahead is to continue to drive forward the Discharge without Delay improvement programme and ensure that multi-agency discharge planning starts as soon as possible in the person ’ s journey , involving all parties from health , social care , housing and third sector , as well as the individual and their family .
Others include continuing to embed human rights in the provision of social care support , ensuring that people have a voice in how social care is delivered , taking a preventative approach , including through the development of the Getting It Right For Everyone ( GIRFE ) practice model , establishing a Social Care Workforce Programme , to continue to deliver the type of improvement the social care workforce , and publishing a Carers Strategy .
