PREPARING FOR YOUR FIRST VISIT
WE ENCOURAGE YOU TO COME PREPARED WITH A FEW ITEMS FOR YOUR
PHYSICAL AND EMOTIONAL COMFORT, SUCH AS:
• Photo ID and insurance cards
• Any test results, X-rays, slides, and other information related to your diagnosis. Request these
from the doctor who made the diagnosis. Please make sure to bring a CD of imaging studies.
• A notebook to record information from your care team and write down questions you might
have during your visit
• This resource guide
• Comfortable, layered clothes; temperatures can vary within the facility
• Any legal paperwork that might be pertinent to your care, such as power of attorney, advance
directive, living will, etc.
• Money for parking and incidentals
• If you are on oxygen, please ensure that you have sufficient oxygen for your travel and visit
with us.
