We’re looking for a passionate and motivated Occupational Therapist
to join our team in the delivery and development of the new Forensic
Mental Health Community Service Model (NHSE 2018) across our forensic
services.
Sussex Partnership NHS Foundation Trust is one of only three national
sites to pilot the new model so this is a really exciting time to join us.
Our focus is on improving the transition of patients through forensic
services, helping to reduce time spent in hospital and better prepare
people for life in the community.
You will:
• Provide a bridging role between hospital and community
• Meet with patients before discharge and work with them until they are
settled in the community
• Work with patients to support personalised recovery plans to help
re-establish purposeful and meaningful lives
• Have a strong focus on delivering high quality evidence based services
that directly support recovery
• Work alongside inpatient and community services, and with
our partners Southdown, to provide expert employment and
accommodation support services
• Travel throughout Sussex (work base is negotiable)
