The below, 2-item Food Insecurity Screen, is based on Questions 1 & 2 of the U.S. Household Food Security Survey and was utilized by all of our Food 4 Health partners to identify food insecure clients within their agencies.

If clients answered often true or sometimes true to either or both of the questions, they were referred to the Food for Health 12-week distribution or prescribed a healthy food box.
