Annual Information Form ( AIF )
All participants must have a 2021 AIF on file to participate in Winter 2022 Programs . The AIF can be found online at
nssra . org / forms .
In the Spring of 2022 we are changing how to submit confidential forms including the Annual Information Form , Seizure Information Form , and the Allergy Form . See page 14 for more information .
Registration Dates
Winter
Registration Dates : November 15 - December 3 Program Dates : January 10 - March 5 , 2022
Spring
Registration Dates : February 14 - February 25 , 2022 Program Dates : March 28 - May 21 , 2022
Program Lottery
A lottery will be conducted for all in-person programs . The lottery will be conducted on December 6 , after which all registered participants will be contacted regarding the outcome of the lottery .
No payment is required at the time of registration ; however , participants who are enrolled in the program will be expected to make a payment by December 10 .
