emax® crowns and #s 22-27 for porcelain
veneers was accomplished. Subsequently all
posterior crowns were cemented. Gold and
ceramo-gold restorations were luted with
RelyX™ (3M ESPE). The maxillary pre-molar
e-max crowns were bonded with Nexus 3 clear
(Kerr). In order to minimize sensitivity, Gluma®
(Heraeus) was utilized for all bonding procedures. With a stable posterior occlusion, jaw
relation records and anterior impression making proceeded routinely. The bonding technique described above was utilized for both the
lithium disilicate mandibular veneers and maxillary full veneer crowns. Several weeks later, a
maxillary night guard was fabricated using the
technique previously described.

Both the esthetic and functional expectations of
this patient were met. Increasing the vertical
dimension of occlusion facilitated restorations
that preserved as much tooth structure as possible. Generally, patients who invest the time
and financial resources for a treatment of this
magnitude comply with follow-up recommendations and achieve stable, long term results.
Treatment Sequence
Utilizing the occlusal deprogramming splint to
maintain vertical dimension of occlusion and
centric jaw relation, the left side posterior teeth
were prepared for full veneer crowns with the
aid of vacuform reduction guides. The preparations were protected with Copalite™ varnish.
The acrylic shell temporaries were relined with
jet acrylic. The same technique was utilized for
the right side posterior teeth at a subsequent
appointment. The next appointment consisted
of jaw relation records and master impressions
of sixteen posterior preparations utilizing Triad®
resin (Dentsply) custom trays, and a hydrophilic addition silicone material, Aquasil®
(Dentsply). At the two following appointments,
