Mount Carmel Health System 2015 Community Benefit Report | Page 2

Dear Community Partners and Friends: Service to others, especially those less fortunate and most in need, is at the core of what defines Mount Carmel as a not-for-profit healthcare system. Our work to provide compassionate, people-centered care in our communities demonstrates our commitment to service, stewardship and, most importantly, our Catholic faith-based mission. We concentrate our outreach efforts on areas where we know we can deliver the greatest good. That’s why our Community Benefit work focuses on the real and urgent needs identified in the Franklin County HealthMap 2013. The Community Health Needs Assessment (CHNA) documented in the HealthMap 2013 is the result of a collaboration of health and education organizations along with community and social welfare agencies. The purpose of this work was to identify and prioritize areas of health care that need improvement based on comparable state and federal data. Mount Carmel works alongside other organizations to implement services and programs to address each area of need highlighted in the assessment, including preventing and treating chronic disease, reducing the infant mortality rate and increasing overall access to care, just to name a few. In this report, we’ve highlighted some of the many activities undertaken in our most recent fiscal year, which addressed these needs. We document Community Benefit information annually to share our dedication to improving the health and welfare of individuals, families and the greater community. This report also is a tribute and special thank you to all of the individuals involved in this work. Thank you for your ongoing partnership and committed support of critical programs and initiatives that directly benefit our communities. Together, we make a meaningful impact on every life we touch. Sincerely, Claus von Zychlin President & CEO Mount Carmel Health System Sister Barbara Hahl, CSC Senior VP, System Mission Mount Carmel Health System 2
