Healthy Ageing Support

We promote healthy ageing through services that strengthen wellbeing , choice , independence and social connections .
HOME CARE TO STAY INDEPENDENT
Our Home Care Packages ( HCP ) support older people who have complex or changing care needs stay living in their homes and community for longer .
Demand for HCP continued to grow and we increased our team to meet demand .
Responding to Aged Care Royal Commission reforms was an ongoing priority and we restructured our team to assist the transition to the new Support at Home program as part of the reforms .
What did we achieve ?
128 new Home Care Packages , up
36 % from the previous year
4 new team members to meet growing demand
ensured continuity of care regardless of the struggles to fill services due to staff shortages across the sector
Challenges
• staff shortages in Aged Care have doubled in the last 12 months impacting the delivery of essential care and services to consumers
• implementation of payment in arrears had significant impacts on processes and systems , initially increasing out-of-system processing
• employment award changes increased costs for consumers and impacted the number and type of services they could pay for in a HCP . Managing this increased the HCP team workload and further strained staffing

• balancing increased demand for HCP essential care and services with Aged Care staff shortages
• meet key milestones related to the implementation of Aged Care Reforms
• rollout new work instructions to support increased efficiencies and workflow continuity
SUPPORTING DEMENTIA AND MEMORY LOSS
Our Specialist Aged Services ( SAS ) team provides integrated allied health and nursing care to help people with dementia manage their changing needs and stay independent for longer .
Redesigning our memory and dementia services model and integrating community nursing to make it easier for clients to get timely , comprehensive care was a focus this year . We also increased collaboration with health professionals and consumers to deliver new services .
What did we achieve ?
2446 hours of memory and dementia support services provided to clients
redesigned and launched a new , improved memory and dementia model of care
launched innovative Assistive Technology Library for clients to trial equipment ;
77 % of loans led to clients successfully integrating the equipment into their routine
HEALTH SERVICES partnered with clients to pilot Jintronix , a new virtual reality exercise program ;
67 % of participants reported they would choose the virtual program over traditional exercises
126 dementia education sessions for carers supporting someone with dementia
