Lately , it seems like the healthcare delivery process is becoming more complicated – for both providers and patients .

Caregivers are burdened with mounting administrative tasks , from tracking and reporting core measures to adopting new standards required by Meaningful Use and other federal initiatives . Meanwhile , hospital patients are often overwhelmed during the process of receiving care . At a time when they ’ re already feeling anxious or stressed , patients must try to comprehend a staggering amount of information , ranging from pre- and post-op instructions to billing and insurance processes .

As health care providers , we understand how important it is to support patients who are transitioning from a hospital stay to their home . But experience has also taught us that the most effective programs for monitoring patients during their care transition are also the most resource-intense – and we ’ re already stretched thin .

To address this challenge , MDnetSolutions has developed a user-friendly , affordable tool that meets the needs of both patients and providers during the critical first 30 days following hospital discharge . Using interactive voice response ( IVR ) technology , our Care Transition System ( CTS ) checks in with patients via a series of short , carefully crafted telephone surveys specific to their medical condition or procedure .
