• To enroll in EZ PAY, simply add( or update) your credit card information on file – this can be done in person or over the phone.
• Your current monthly balance will be charged to the card on file on the 20th of each month.
MEMBER BILLING: Billing Inquiries, Statement Disputes, Medical Leave Requests and Cancellations must be submitted in writing to Billing @ Eaglebrooke. com.
