Living Well with Chronic Conditions
Living Well with Chronic Conditions ( LWCC ) developed by researchers at Stanford University , empowers older adults who have chronic conditions - or their caregivers - to recognize and carry out health strategies that help them better manage everyday life . Research has found that people who complete a LWCC workshop feel healthier , report a better quality of life , experience fewer sick days , have less depression , are better able to manage symptoms , and more .

Following preregistration , a representative from Faith Community Nurse Network will contact you to complete the program registration process , which includes demographic and health care information . National Diabetes Prevention Program ( NDPP )
The National Diabetes Prevention Program ( National DPP ) was created in 2010 to address the increasing burden of pre-diabetes and type 2 diabetes in the U . S . One key feature of the National DPP is the CDC-recognized lifestyle change program , a research-based program of healthy eating and physical activity which showed that people with prediabetes who participate in a structured lifestyles change program can cut their risk of developing type 2 diabetes by 58 % ( 71 % for people over 60 years old !).

There are two information session for participants to ask questions about the program and requirements and for Live 2 B Heathy staff to gather required information to complete the registration process . Attendance at one of the two info sessions and submission of required information is required prior to attending the first class on April 27 . Required information includes health insurance and blood glucose test results .
If you wish to participate in the program but do not qualify as prediabetic through your insurance , or if you wish to participate as a support for a family member of friend , but you yourself are not prediabetic , there is a $ 100 fee to attend . This fee is to reimburse for class materials that would normally be covered by participants insurance .
Class meets weekly for the first two months , then twice a month for four months and then monthly for the last six months - making this a full year commitment sure to bring results .
