2) Care Management for All: A Post-Acute
Care Provider Perspective
This program will focus on the need for
effective care coordination by providers with
managed care plans to achieve optimum
outcomes for plan members. In this program
we will provide participants with the
methods and strategies to build an internal
care coordination program to partner with
managed care plans. Topics will include a
discussion of the core components of an
effective care coordination program such as,
data systems and analysis, clinical practice
guidelines, transitions in care, prevention
of avoidable hospital readmissions and
establishing communication systems to
support provider/payer relationships.

5) Improving Outcomes in Assisted Living
During this session, you will learn about
how an Assisted Living Program (ALP) has
developed a comprehensive and integrated
approach to resident centered care and
service delivery. By maximizing their own
resources and collaborating with other
community providers, the ALP has been able
to improve outcomes for their residents,
including a reduction in unnecessary
hospitalizations and frequency of falls. The
presenters will share information to help
participants consider how they may utilize
similar strategies in their own adult care and
assisted living facilities.
