YOUR Pathway Plan
Your pathway plan is a legal requirement and a contract between yourself and the Leaving Care Service about what we will do to support you to reach your goals .
The plan is drawn up with you over a period of time and we will review it with you every 6 months . We have listened to your feedback and have changed the format of the pathway plan to make it more straightforward and clear .
When your worker visits you they will talk to you about the various parts of your plan and how you would like things to progress .
The areas it will cover are accommodation , education , training and employment , your safety , finance , health , identity and family / social networks .
The new pathway plan also includes a section on equalities , equity and empowerment . The purpose of this section is to ensure we discuss any discrimination you may be facing and plan with you how to resolve the issues . We added this section based on the care leavers equalities survey you completed .
We are planning to have reviewing officers to oversee the pathway plan reviews held for 18-year olds from Summer 2021 .
The pathway plan and reviews ensure that the Leaving Care Service carries out the actions that we have agreed together so that the plan works well for you .
