Health center providing healthcare and
dental services to the community
T
he Jane Pauley Community
Health Center was established
in 2009 with generous support
from the Metropolitan School District
of Warren Township, Community Health
Network and the Community Health
Network Foundation. It is named after
Jane Pauley, a 1968 Warren Central High
School graduate who grew up in the area
and is well known as the former anchor
of NBC TV’s “Today” and “Dateline”
programs. She currently hosts CBS
“Sunday Morning.”
The Jane Pauley Community Health
Center offers comprehensive health care
to the communities we serve, regardless
of a patient’s ability to pay. We are
currently accepting new patients at our
Shelbyville location, where we offer
primary healthcare and behavioral health
services for adults and children. Our
services include preventive care, annual
exams, well-child checks, acute care and
certain procedures. Our providers also
focus on the management of chronic
diseases, such as diabetes, cardiac disease
and depression.
Getting in for an appointment with
The Jane Pauley CHC at Shelbyville
is fast and easy. Whenever you need to
see a healthcare or behavioral health
provider, get your child immunized,
or receive another type of medical
service, call us at 317-421-6060 to
schedule an appointment. We offer
convenient appointment scheduling to
meet your needs and will schedule your
appointment as quickly as possible.
Currently, we are scheduling for same-
day and next-day appointments. Our
Shelbyville location is open Monday
through Friday from 8 a.m. to 5 p.m., as
well as some Saturdays from 8 a.m. to
noon.
The goal of our organization is to
provide quality healthcare and dental
services at an affordable price for all
members of the community. We accept
many types of commercial, Medicaid,
Medicare and Marketplace insurance
plans. If you do not have insurance, we
have an on-site Indiana Patient Navigator
who can work with you to see if you
qualify for insurance or for our Sliding
Fee Scale. Prescription assistance is also
available for those who qualify. We are an
Enrollment Center for Indiana Medicaid,
Healthy Indiana Plan (HIP 2.0) and the
Marketplace.
The Jane Pauley Community Health
Center has sixteen sites in four counties
offering care to patients of all age groups,
nationalities and backgrounds. Three
of our locations offer dental services to
those who do not currently have dental
insurance. For both our dental and
behavioral health services, anyone can
establish care, even if you have a primary
care provider outside of The Jane Pauley
Community Health Center.
