SHARON MCDERMOTT-HEAPS, NOVEMBER 2015 SUPPORTING OUR PATIENTS BEST: SHORT STAY UNIT 72 hours of patient care 8 Joseph Brant Hospital JBH regularly sees patients who medically do not require hospital care but complex life conditions mean they cannot immediately return home, so they are admitted to hospital. This year, we identified this situation as something we can improve for our patients. JBH partnered with the CCAC to develop a Short-Stay Unit pilot program - providing up to 72 hours of care for patients while working intensely with their families and the CCAC to arrange the best community support programs so patients do not have to be admitted to hospital. With its success, JBH will continue to evolve the short stay initiative and have identified this is a key strategy that can be implemented to help address Alternative Level of Care (ALC) patients in the upcoming year.
