for Burlington:
John’s Story
In Burlington, there are numerous agencies and
services that support the health care needs of our
community, along with our hospital. While each
program has their individual vision and purpose,
when these passionate and driven groups come
together in a united setting, their passion to care
for our community truly shines.
Our Local Heath Integration Network (LHIN)
created six “Anchor Tables,” dividing our region to
engage the health planners and providers of each
diverse community. Burlington is home to one of
the six Anchor Tables, representing our community
as the Burlington Anchor Table.
The purpose of the Burlington Anchor Table is to
come together to plan, connect and collaborate
on the health and wellness of the Burlington
population. The table is co-chaired by Dr. Cheryl
Williams, Joseph Brant Hospital Executive Vice
President Clinical & Chief Nursing Executive and
Kathy Peters, Joseph Brant Hospital, Director,
System Collaboration and Partnerships. It includes
diverse representation from the public, health
institutions and services, home and community
care agencies, and local, regional and provincial
government. General members of public also
participate in an Advisor Role to share lived
experience, knowledge and insight.
“I became involved with the table after they
did an outreach to the community asking for
people to join” shares John Reynolds, Patient and
Family Advisor. “There were two tables they were
recruiting for, and I was privileged enough to be
selected for the general table, so-to-speak.”

The Burlington Anchor Table has focused its
attention on the care of older adults, mental
health and addictions, and complex care.

group. They create hope and optimism
that some things will change.”
“They are working on a new model for
developing communities for seniors that provide
for housing and care in a single setting,” John
shares. “They have listened to the feedback
and considerations we have provided about
engagement activities and key timelines. It’s
better to reach out to these communities now,
rather than later.”
Through the Burlington Anchor Table, Joseph
Brant Hospital is delivering on its commitment
to engage in collaborative planning efforts.
We contribute to the goal of providing more
streamlined and patient-oriented services to
ultimately realize better health outcomes for our
patients and community.
