President
As we approach the final months
of 2017, we have much to
look forward to and celebrate.
November is designated as
National Hospice and Palliative
Care Month, and Dec. 5, 2017 is
Colorado Gives Day, the largest
giving event in northern Colorado throughout
the year.
In honor of National Hospice and Palliative
Care Month, I’d like to take this opportunity to
remind our community of the extensive hospice,
palliative care, and bereavement services
Pathways offers to ensure the utmost care
during life’s difficult times.
Pathways was recently ranked as the number
one hospice organization in northern Colorado,
according to the Hospice Quality Reporting
Program by Medicare.gov. These results are
released quarterly, and we look forward to
seeing our quality increase as the months
continue into 2018. In 2016 alone, our
dedicated team of medical professionals cared
for more than 1,200 hospice patients and
saw more than 1,500 palliative care visits
throughout last year. Although differing in
eligibility criteria and scope of services, both
programs include symptom management,
emotional and spiritual support, and respite
for caregivers.
Because Pathways has always placed great
emphasis on honoring life throughout every
step of the way, we chose “LIVINGtoday” as our
Colorado Gives Day theme. We encourage our
community to ask themselves what motivates
them to live well and consider how they are
living today.
Colorado Gives Day is an excellent opportunity
to embrace the giving spirit of the holiday
season. As the president of Pathways, I ask that
you remember Pathways and the community we

Community Connections
Through Alzheimer’s Walk
The Walk to End Alzheimer’s gathers members of our community to take
a stand against this terrible disease and fight for the advancement of
Alzheimer’s support, care and research. This year, our very own Christina
Wilkening of the Provider Relations team participated on the 2017 Greeley
Walk to End Alzheimer’s leadership committee. This allowed Pathways to
build and strengthen our connections with several facilities in Weld County,
including Meadowview of Greeley and Fairacres Manor in Greeley.
Meadowview has used Pathways as a hospice provider for the past few
months, and we’ve greatly enjoyed our ongoing relationship with them.
Meadowview is dedicated to providing premier senior living with high-
end amenities for residents, including spacious apartment floor plans,
restaurant-style dining and life enrichment activities. Pathways has been
happy to support many of their community events over the past year, such
as their carnival and trivia night.
Fairacres Manor is another spectacular facility that Pathways has worked
with through the Greeley Walk to End Alzheimer’s. Fairacres cares for those
in need of post-acute rehabilitation, long-term care or secure memory
support. They are proud supporters of the Walk and have been a part of the
committee for four years. We appreciate their community-oriented values
and look forward to partnering with them in the future!
The collaboration among several organizations for the 2017 Greeley Walk
to End Alzheimer’s has been phenomenal. Through excellent participation
and leadership, the Walk has raised more than $85,000, astronomically
surpassing its original goal of $52,000. It is an honor to be a part of this
important cause and aid in the fight against Alzheimer’s.
Pathways Ranked
#1 Hospice Organization
in Northern Colorado
Pathways is pleased to announce that it was recently ranked as
the #1 hospice organization in northern Colorado, according to the
Hospice Quality Reporting Program by Medicare.gov. We have always
had a strong reputation within Larimer and Weld Counties for serving
community members during their times of need, and we are proud
that our tireless efforts have been recognized in this recent ranking.
We look forward to our numbers only going up in 2018.
