Appropriate Form G to be Used in Support of CCC as Prescribed under UBBL
BEM noticed that the standard Form G used by Majlis Daerah Sabak Bernam ( MDSB ) bears its letterhead and logo . BEM had consulted with Jabatan Kerajaan Tempatan ( JKT ) with reference to the prescribed standard form in the UBBL . JKT in it ’ s letter cautioned MDSB that Form F and Form G used for the issuance of CCC should follow the gazetted UBBL , in this case Undang-Undang Kecil Bangunan Seragam Selangor ( Pindaan ) 2007 .
The JKT letter to MDSB is appended herewith for reference .
