Water depth , c . Wave height , d . Wind speed , e . Total dive hours , f . Breakdown of diving activities from the surface and return with time spent .
7 . Shift handover report – a report passed between day and night supervisors . 8 . Tool box talk report – prepared at the start of each shift . 9 . Incident report – if any incident happened 10 . Maintenance log . 11 . Scheduled / Weekly maintenance log . 12 . A material requisition form – if there is a requirement for additional or replacement tools , hardware , stationery , consumables or other requirements . The form is sent to the base office for purchase .
During diving operations , videos are recorded and are submitted to the client at the end of project .

THIRD PHASE At the end of each project , a ROV supervisor must ensure all the following tasks have been completed :
1 . He has informed base office as soon as possible , regarding the demobilisation date so that the following agenda may be arranged : air tickets , accommodation if needed and logistic agent to standby for return of ROV system .
2 . He has sent an end of trip report to the office covering : an evaluation of the operation , and a technical evaluation of the ROV system .
3 . He has completed a demobilisation checklist to ensure the crew have not missed anything , including : a . All connected hoses and cables have been removed from the systems , b . The ROV has been washed with clean fresh water to remove all dirt and salt , c . All the systems have been secured onboard the vessel / oil rig so that there is no movement during transit , d . The ROV has been covered with a tarpaulin sheet , e . All containers have been locked and the keys passed to the person in charge on board the vessel / oil rig . The key will then be passed to the logistic agent to be passed to the base office .
