Post-Hospital Care &
Rehabilitation Options:
You Have a CHOICE!
R
emember the days when you stayed
in the hospital until you completely
recovered?
With healthcare constantly changing,
hospitals today handle acute cases,
then transition patients (especially older
adults) to post-acute care for recovery and
rehabilitation. The doctor or hospital can
recommend options, but the choice is up
to you!
Most Medicare plans cover short-
term rehab stays in nursing homes after
injury or illness, which can be benefi cial
for older adults. This setting is more
convenient than traveling to and from
an outpatient center—especially if the
injury is orthopedic, where traveling to
appointments risks re-injury. You might
think that being away from home may be
stressful; however, studies show that a more
successful recovery is achieved with 24-
hour supervision and treatment since you
can rest assured knowing someone is there
to help at any time. Short-term rehab can
be more favorable than utilizing a home
care agency, especially for people who
live alone, do not have a 24-hour caregiver
available, or have a home with stairs or an
inaccessible layout.
Sarah Reed off ers short-term rehabilitation
in our beautifully updated Skilled Nursing
facility. Residents benefi t from comfortable
private accommodations. Services include:
individualized care plans, 24-hour nursing
care, six days of therapy per week, meals
under the direction of a registered dietician,
activity programs, and discharge planning.
Another option for recuperation is a
“respite” stay in a Sarah Reed apartment.
This is ideal for someone who may not need
extensive nursing care or therapy. Instead,
the person would temporarily reside in a
furnished apartment with an emergency pull
cord, receive three meals per day, and have
individualized nursing services as needed.
In some cases therapy can be added. This
PRIVATE ROOMS
option can be utilized as a step-down from
skilled nursing before transitioning to home.
A respite stay is a great way to sample the
Sarah Reed lifestyle if you are considering a
senior living community.
What types of conditions can be treated
during a short-term rehab stay? Most injuries
needing rehabilitation are orthopedic,
neurological, cardiac and respiratory.
Treatments involve recovering mobility,
rebuilding muscle function, strength training
for utilizing walkers and wheelchairs,
improving communication skills, and
providing the emotional support needed
to cope with the defi cits and resume daily
activities like bathing, dressing, and meal
preparation.
Discussing rehabilitation options with
your doctor is important if you have health
issues. Additionally, the hospital social worker
or case manager can help you fi nd a rehab
facility that is right for you.
Remember, you have a choice of where
to go for rehab. It is wise to research and
visit facilities before a need arises. You will
be able to make pre-arrangements for rehab
after a planned procedure, and in case of
an accident or sudden illness, you would be
prepared to VOICE YOUR CHOICE!
