KINDERGARTEN REGISTRATION
For children who will be 5 years old before September 1 st
Kindergarten registration for the 17-18 school year will be held at the school where your child will attend on the dates listed below. t

Monday, May 8 th Tuesday, May 9 th Wednesday, May 10 th Monday, May 15 th Wednesday, May 17th
Times are 9:30-11:00 a. m. and 5:00-7:00 p. m.
Registration packets will be available to complete at the school and are available on the Student Registration link of our website: www. mtsd. org. These sessions are for registration only. Students need not attend.
REQUIRED DOCUMENTATION- The following documentation is REQUIRED. Registration cannot be completed unless all is provided. illcreek

• Original Birth Certificate
• Updated Immunization Record
• All documents in the registration packet, completed, signed and dated
• Custody Agreement( If applicable)
• Proof of residency in Millcreek: Parent MUST provide three( 3) documents with the same address: 1. Valid PA Driver’ s license, PA Identification card or Address Update card
-AND- 2. Public Utility bill such as Penelec, National Fuel or Water / Sewer bill
-AND- 3. Renters: Lease agreement( signed and dated) 4. Home Owners: Property tax bill, Deed or Mortgage Closing Disclosure
Note: Bank or mortgage loan statements, phone bills or temporary driver’ s licenses are NOT acceptable proofs.
