H Our membership drive ended November 30 , 2016 , however you can still purchase a membership anytime throughout the year .
H Memberships cannot be pro-rated . The purchase of a one-year individual membership is $ 35.00 , with a $ 5.00 discount , for an individual over the age of 65 . The purchase of a one-year family membership is $ 45.00 , with a $ 5.00 discount , for a family over the age of 65 . ( All individuals included must be over the age of 65 to receive the discount ). This membership is valid when payment is received and is valid until November 30 , 2017 .
H The purchase of a two-year individual membership is $ 60.00 , with a $ 5.00 discount , for an individual over the age of 65 . The purchase of a two-year family membership is $ 80.00 , with a $ 5.00 discount , for an individual over the age of 65 . ( All individuals included must be over the age of 65 to receive the discount ). This membership is valid when payment is received and is valid until November 30 , 2018 .

Non-Emergency Transportation Services
At Millcreek Paramedic Service , we pride ourselves on ensuring the health and well-being of our patients .
Our specialized wheelchair van transportation was established over 16 years ago , in Millcreek Township .
MPS provides medical transportation on a non-emergency basis . This includes hospital to hospital , home to hospital , or hospital to home . We maintain six wheelchair vans that are capable to transport , on a non-emergency basis , wheelchair bound patients to and from hospitals , clinics , and home . We also offer transportation for dialysis , wound care and cancer center treatments . Those with an ambulatory dysfunction needing assistance to and from personal gatherings can also schedule transportation . These include , but are not limited to , weddings , funerals , family and holiday gatherings and any other type of ongoing assisted transportation needs .
Medical Taxi service provides transportation of individuals that are ambulatory but cannot or choose not to drive for any reason .
By calling our business line at 814.836.8677 , you will be able to schedule transportation day or night . MPS strives to provide prompt transportation to your appointments . We encourage our consumers to schedule as soon as possible to better accommodate your transportation needs .
Our trained staff remains available to assist you to and from our vehicles and assure safe transportation to your appointments .
Questions often arise as to whether you Millcreek Paramedic Service membership entitles you to free or reduced wheelchair service .
In accordance with regulations set forth by the Office of Inspector General , U . S . Department of Health and Human Services , certified by the Public Utility Commission , we are not legally allowed to offer vouchers or special rates based on ambulance membership . Our
EMPLOYEE OF THE QUARTER wheelchair transportation service is a billed service . We will not ask for payment upfront , the patient will always be billed for services after the transport has occurred . Rare circumstances occur where a longdistance transport is needed ( outside of Erie County ), payment for these particular transports will normally be required upfront , unless otherwise specified by the billing manager .
Most insurance plans , including Medicare and Medical assistance , and their HMO plans under this umbrella of coverage , do not cover the expense of wheelchair transportation .
Whether it be due to inclement weather , special personal needs , or lack of transportation , please keep Millcreek Paramedic Service in mind for any wheelchair transportation needs in the coming months and throughout the year .
IN RESEARCHING TRANSPORT SERVICES , YOU WILL FIND THAT MPS RATES ARE COMPARABLE TO OTHERS IN OUR AREA .
TRANSPORTATION RATES :
• $ 35.00 One way transport
• $ 60.00 Round trip transport
• $ 4.00 per mile after 10 miles
• $ 20.00 per half hour of wait time after 30 minutes
• $ 20.00 for an additional MPS employee to assist with any special needs for a one way transport
• $ 30.00 for an additional MPS employee to assist with any special needs for a round trip transport
EXAMPLES OF SPECIAL NEEDS :
• More than four ( 4 ) steps
• Weight exceeding 200 pounds
• Behavior Health Care needs
• Patients under the age of 18 without a parent or guardian
Congratulations to Andrew Carne , selected as Millcreek Paramedic Service ’ s “ Employee of the Quarter .” Andrew is an Emergency Medical Technician and Team Leader . In addition to his normal duties , Andrew has developed updated training standards for new employees at MPS , and acts as the coordinator of the MPS New Employee Orientation program . Andrew has been a vital asset to MPS , as he has worked to set up MPS on the Erie County 911 messaging system , known as “ I am Responding ”. Beyond his dedication to service at MPS , Andrew is currently a Captain with the West Lake Fire Department . We are proud to have Andrew as part of our team ! illcreek
