CV Kindergarten Pre-Registration
for 2018-19 School Year
The Primary School is requesting that parents/guardians pre-register children who
will be eligible to attend Kindergarten in the fall of 2018. Below is a printed form for
pre·registration. Complete the form and mail it to the Primary School. A notice regarding
time and date for registration along with a registration packet will be forwarded to those
who complete and return the pre·registration form.
A child must be 5 years of age on or before September 1, 2018 and be properly
vaccinated before the start of the school year to be enrolled in Kindergarten. Proof of the
following immunizations must be presented before the child can enter school.
Diphtheria- 4 doses of toxoid, one dose given after age four
Tetanus- 4 doses of toxoid, one dose given after age four
Polio- 4 doses of vaccine (IPV or OPV), one dose given after age four
Measles- 2 doses of vaccine, first dose administered at 12 months or older (usually MMR)
Mumps- 2 doses of vaccine, first dose administered at 12 months or older (usually MMR)
Rubella - 1 dose of vaccine, administered at 12 months or older (usually MMR)
Hepatitis B - 3 doses of vaccine, 3rd dose must be given after 6 months of age
Varicella - 2 doses of vaccine, first dose administered on or after first birthday or a doctor’s
written statement of history of child having had varicella (chicken pox)
***Proof of lead levels in blood is a new requirement beginning January 1, 2018.
KINDERGARTEN PRE-REGISTRATION FORM
We are interested in determining how many children will be entering Kindergarten next
September. This will assist with our planning for the 2018 • 19 school year. If you have a
child who will be 5 years of age on or before September 1, 2018 please complete the
form below and return to the Primary School by February 9, 2018. You will receive a
packet of enrollment forms to complete before your registration appointment. Official
registration will be held on the evenings of March 13th and March 141h. You will be
contacted by the secretary to schedule your specific day and time.
