There are a few documents that you need to bring to your
USCIS medical doctor in order for him to be able to
complete your file and perform your immigration
physical.
You are required to bring:
 Form I-693, which is the Report of Medical Examination
and Vaccination Record
 A government-issued ID (children under 14 can bring
a birth certificate or an ID that shows his name, date
of birth, place of birth, and parent’s full name)
 Immunization records
 Your insurance card
 The doctor’s fees
