Enter 5 POINTERS FATAL FATIGUE A Metro North train derailed on Sunday in New York, killing four people and injuring more than 70. On Monday, federal investigators revealed the train was traveling 82 miles per hour through a dangerous turn in the track, more than three times the speed allowed in the area.
