Always choose in-network providers to get the most out of your benefits! Seeing an out-ofnetwork provider means higher costs or denied claims. If you see an out-of-network provider, you could be balance billed— meaning you may have to pay the difference between what your plan covers and what the provider charges. Using an in-network provider protects you from these extra costs.
FINDING AN IN-NETWORK PROVIDER IS EASY!
How to access: 1. Go to HometownHealth. com / provider _ directories.
2. Select your network. You can verify your network on the upper left corner of your ID card.
3. Once you click on your network, the directory will automatically be filtered to show your in-network providers. It’ s that easy! Then you can search by name, specialty or location to find the best provider for your care.
