What is a Comprehensive Health Assessment?
The Comprehensive Health Assessment is a special visit designed to help you get the most out of your health plan benefits. Think of this as your” one-stop shop“ for getting set up for the year. During this appointment, our team will take extra time to understand your health history, current needs and goals for the year ahead. Our friendly medical team will review all of your health needs and share them in a summary to your Primary Care Provider so they have the most up-to-date information about your health. During your Comprehensive Health Assessment visit, you can expect:
• A full review of your medications and health conditions • Time to complete or schedule preventive screenings and complete a physical exam
• A check on your emotional, social and home-based needs
• Help connecting you with additional programs such as Care Management
How is it different from seeing my Primary Care Provider( PCP)?
• Your PCP focuses on diagnosing and treating medical conditions and managing your day-to-day care.
• The Comprehensive Health Assessment visit focuses on prevention, coordination and uncovering any unmet needs – so your Primary Care Provider can spend more time on treatment knowing that we’ ve helped fill in the gaps.
• After your visit, we share a summary with your PCP to keep your care team fully informed.
Together, your Comprehensive Health Assessment Team and PCP work hand-in-hand to keep you healthy, supported and connected to the care you need.
