You will need to submit diagnostic confirmation of the diagnosis. This form can be found on the CurePSP Voice Program page, in the information page on the ElevenLabs website and in the email application. Hit send.
Applications without the diagnosis verification will not be approved.
Be sure to go back to the application page and select“ DONE- I’ ve Sent the Email!
