Written Formal Complaints ( Any written complaint from providers will be categorized as a Formal Complaint ):
1 . Claim disputes see “ Claims Reconsiderations ”
2 . Credentialing / Recredentialing disputes see “ Credentialing / Recredentialing Denial Dispute Resolution ”
3 . For any non-claim , related disputes please email
HTHProviderRelations @ hometownhealth . com . A response will be sent to the provider or third party entity no later than 30 calendar days from receipt of the complaint , or as dictated by the state regulation .
