EMPLOYEE HEALTH
Why:
Unwell food handlers can transfer illness to others.
Who:
All employees.
When:
When employees are hired and throughout employment.
Where:
Throughout the establishment.
How:
Manager must complete the “Employee Health Questionnaire” and
“Employee Health Reporting Agreement” with all employees.
Employees must report any flu-like symptoms, diarrhea, jaundice, lesions
and / or vomiting.
Hepatitis A, Salmonella typhi, Shigella, or shiga toxin producing E. coli
must be reported.
Food handlers with discharge from the ears, nose or eyes must not handle
vehicle interiors.
Medical release to return to work if one of these illnesses

“Employee Health Questionnaire”
“Employee Health Reporting Agreement”
