The following table displays the full range of responses relating to the implementation of the Provider Data Management initiatives . Similar to the 2019 and 2020 results , the difference between the highest and lowest percentage of unsuccessful implementation was small . The percentages of unsuccessful implementation varied from 5-9 % across all provider initiatives in 2019 , but in 2020 the percentage range was even smaller ( 4.7-7.5 %). This year , the percentage range of unsuccessful implementations spanned from 4.9-8.1 %. Consistent with results from 2018 , 2019 , and 2020 , the highest rated initiative for nonimplementation remained the same . In 2020 , nearly four in ten ( 39.4 %) reported they did not attempt to add data to provider databases , and in 2021 this increased up to 40.1 %.
