COORDINATION OF CARE: ACCESS TO A MEDICAL HOME AND BEHAVIORAL HEALTH HOME; COORDINATED NETWORK OF HEALTH AND HUMAN SERVICES Goal: Increase the Number of Residents in Lake County Who Have Health Insurance A certified navigator with Enroll Lake County helps a client apply for health insurance. I n July of 2013, the Health Department launched the Enroll Lake County! project, a countywide effort to help residents access new forms of health insurance made available through the Affordable Care Act. The Health Department and several partners oversaw licensed health insurance navigators who provided education and application assistance to individuals interested in applying for Medicaid or purchasing insurance through the federally-facilitated Health Insurance Marketplace. In its first year, 26,717 Lake County residents enrolled in healthcare coverage. During the second year, the number of residents gaining health coverage through the ACA rose to 42,273 individuals, with 21,586 residents enrolling or re-enrolling for coverage through the Marketplace and 20,687 residents enrolling in ACA Adult Expanded Medicaid. While the impact of the ACA and Enroll Lake County! efforts are still being analyzed, the uninsured rate in Lake County dropped from 11% in 2013 to 6% in 2015.
