How Do I Register?

Selec�ng a Class
Choose a class from the chart on page 4 based on your child’ s age as of September 1, 2016. This date is used to determine eligibility for classes for both Semester I and Semester II.
Immuniza�on Records
Immuniza�on records MUST accompany the registra�on form if your child is par�cipa�ng in a parent-child class or sibling care. Class registra�ons will not be processed without an up-to-date immuniza�on record each school year. Immuniza�on records can be faxed directly from your clinic.

Determine Payment Due
Use the class fee schedule on page 5 to determine fee for the first child a�ending the class. Add half the fee for each addi�onal child a�ending the same class. List any children who will need sibing care and add sibling care fee. Include full class fee and sibling care fee in total fees due.
Payments
Payment for Semester I is due at the �me of registra�on. Payment for Semester II is due December 16. No District 622 family will be denied par�cipa�on in parentchild classes due to inability to pay fees. Fee assistance or payment plans may be arranged by calling 651-702-8411.

Class Confirma�ons
If you registered online or provided an e-mail address on your registra�on form you will receive an e-mail confirma�on. You will be no�fied by phone only if you did not get into the first-choice class you registered for.
Cancella�ons
Classes are cancelled if registra�on does not meet minimum requirements by September 1 for Semester I and December 16 for Semester II.
Changing Classes
A $ 15 fee is charged for changing classes.

Refund Policy
Refunds will be issued when requested before the registra�on deadline or at least five working days prior to the class star�ng date. A $ 15 service charge will be deducted from each refund processed. We reserve the right to cancel classes due to insufficient enrollment. Fees paid will be refunded. Refunds will not be issued for classes missed by par�cipants or a�er a class has started. Class cancella�ons due to weather condi�ons are not refundable and will not be made up.
